If you have not done so already, please could you logon and complete the consent form. As this is run by the School Nursing Team, we are unable to do this in school for you.
A letter has been emailed to all parents containing the web address to access the online consent form. It includes all the information you will need, including the school’s unique code. This needs to be completed before Thursday 10th of December.
